A member of the Community Pharmacy Patient Safety Group shared details of an incident in which a team member inadvertently revealed the new address of children to their father who had come into the pharmacy to collect his prescription. Whilst adverse outcomes were avoided, the incident could have had safeguarding implications and learnings are necessary.
To ensure learnings are widely shared the Patient Safety Group has developed a “Report, Learn, Share, Act, Review” resource.
