Concerns have been raised with the Patient Safety Group about a number of handout errors, in which methadone has been supplied to the wrong patient. The root cause of such incidents is often over familiarity.
In response to these concerns, the Patient Safety Group has developed a resource which encourages pharmacy teams to learn from such incidents and highlights best practice when dispensing methadone.
