The Patient Safety Group discussed an incident that was brought to the Group’s attention regarding an administration error of Citalopram 40mg/ml oral drops by a carer.
The patient received a dose 10 times greater in volume and strength than the prescriber intended.
In response to this incident the Patient Safety Group has developed a resource to support learning and highlights best practice.
